Case Management Services provides support and assistance for youth and families involved in Travis County Juvenile Probation. Case Management services are home and community-based services for youth and their caregivers delivered during a period of four months. Independent Case Management Services works to alleviate personal, family, and environmental stressors that negatively impact behavior and social development.? Many of these are stressors that diminish chances for success in the community. We assist youth and caregivers in developing strengths, leveraging resources, and addressing unique needs so that youth are able to complete the term of probation successfully. We directly link youth and family members to services and opportunities that support growth and stability.? These resources include housing, healthcare, basic needs, employment, academic, financial, recreation, immigration assistance, transportation, parenting classes, among others. Through linking these services, we create a network of treatment and other services that will provide a continuum of care for substance abusing offenders.
Teens age 13-17 who are currently in the Travis County Juvenile Probation and have been adjudicated, are eligible for four months of ICMS case management services.?Program procedures allows the juvenile probation officer complete a service referral form and submits it to the lead probation officer for review.
